Teaching HIV Prevention in the HIV Clinic

The purpose: to develop and evaluate intervention strategies within the HIV medical care setting to addressthe prevention needs of HIV-seropositive persons through changing the practices of HIV care providers(physicians and nurse practitioners).

The primary objectives of the intervention: to reduce high-risk sexual HIV transmission behaviors among HIV-infected persons receiving care in HIV medical clinics, to assess barriers and facilitators related to the delivery of HIV prevention counseling within the HIV medical care setting, and to document the adoption of HIV prevention practices and services by providers and clinic a dministrators.

The overall study hypothesis: HIV-positive individuals in the Experimental Intervention Condition, when compared to those in the Standard of Care Condition, will report greater reductions in high-risk sexual behaviors at follow-up

Study Components:

Phase 1: pre-intervention component in which face-to-face interviews are administered in the HIV medical care clinics with HIV-positive patients to document provider practices regarding the delivery of prevention counseling to their HIV-positive patients.

Phase 2 (Randomized Trial): two-arm, randomized prospective trial of a prevention intervention for HIV-positive individuals receiving HIV primary medical care.

Experimental Intervention Condition:

  • Computerized risk assessment of patient’s HIV transmission risk behavior
  • Risk assessment is used to develop tailored prevention message to be delivered by provider
  • 5-10 minute clinician-delivered and participant-specific HIV transmission risk reduction intervention
  • 30-45 minute HIV peer educator-delivered intervention.

Standard of Care Condition: involves standard clinical practice and services.

Follow-up assessments will be conducted at 3, 6, and 12 months post baseline.

Recruitment: Participants will be recruited from four local HIV primary care clinics

  • UM/JMH Special Immunology Clinic
  • UM/JHM OB/GYN Clinic
  • Miami VA Medical Center
  • PHT/Miami Beach Prevention Education & Treatment Center
